Certification & Release

Volunteer Coaching Waiver

Applicants are considered for volunteer positions for which they have applied without regard to race, religion, sex, age, national origin, disability and other characteristics protected by law. Certification and Release I certify that all statements made in this application are true and complete. I authorize the City to investigate all statements made as apart of this application and to secure any necessary information from all prior employers, references, academic institutions, law enforcement agencies, other persons and entities, and public records. I hereby release all such persons, entities, employers, references, institutions, agencies and the City from any and all liability arising from their giving or receiving information about my employment history, academic credentials, qualifications, reputation, driving record, and criminal record. A photocopy of this release can be used for all purposes. I understand that any false answers or misleading statements as well as misrepresentations by omission made by me as part of my application, will be sufficient for rejection of my application or for my immediate discharge, should one be discovered after I am employed. I understand that nothing in this volunteer application, in the City’s statement of personnel policies or in my communication with any City employee or official is intended to create an employment contract between the City and me. Accordingly, either I or the City may terminate my employment at-will at any time with or without cause or notice. I understand that the at-will nature of the employment relationship can only be changed in a specific writing signed by the Chief Human Resources Officer. I understand that my phone number will be given to players on my team and listed on the game schedule handed out to the league participants. I hereby acknowledge that I have read, understand, and agree to the preceding statement.

I promise to take due care during my volunteer activities and am aware of the hazards and risks that may be associated with my participation in volunteer activities with the City.  I understand, accept, and assume all such hazards and risks and waive all claims against the City of Fort Collins, its officers, agents, volunteers and employees for any injuries suffered by me or damages to my property arising out of or related to my participation in volunteer activities with the City.  Further, I agree to indemnify the City of Fort Collins, its officers, agents, volunteers and employees against, and be responsible for, all harm, injury or damage I willfully and wantonly cause to any persons, property or equipment in conjunction with my participation in such volunteer activities with the City.

The risks and hazards that may be associated with my participation in volunteer activities with the City include the following:

1.    Heavy Outdoor Duties-- heavy lifting, kneeling, standing, stooping, walking or moving over rough terrain, animal bites, kicks, injury from use of hand tools (shovels, rakes, wheelbarrows), planting trees, exposure to stinging or poisonous wildlife/plants, sometimes limited cell phone reception, exposure to extreme temperatures, sunburn, dehydration, cold.

2.    Moderate Outdoor Duties--lifting, small tools, pulling weeds, gardening to include lifting bags of soil, bike riding over rough terrain, crossing roads, bike/pedestrian counts, picking up trash (possible exposure to  trash, human waste, drug paraphernalia, weapons etc.), exposure to stinging or poisonous wildlife/plants, interacting with the public, exposure to extreme temperatures, sunburn, dehydration, cold.

3.    Light Outdoor Duties-- special event/festival, interaction with members of the public, doing crafts, setting up tents, distributing information, crafts/activities with kids or adults, moving supplies, unloading/loading vehicles, set up, take down, exposure to extreme temperatures, sunscreen, dehydration, cold.

4.    Moderate Indoor Duties--- Indoors, front desk or public facing work, providing information, light bending, stooping, kneeling, standing/walking long periods of time 

5.    Light Indoor Duties-- computer work, research, data entry, filing, minute taking, mailings, etc.

City staff or other volunteers may take photos of volunteer activities for City marketing purposes unless a volunteer provides written notification to the City that he or she does not authorize use of volunteer’s photos for this purpose.

I will not drive a vehicle to conduct City business unless authorized to do so by my City supervisor. I understand that I must report all traffic violation citations received while driving on City business, or any suspension or revocation of my driver’s license, to a City supervisor immediately.  Prior to conducting any City business in my automobile, I will secure and maintain all State of Colorado insurance requirements and hold a valid Driver’s License any time I conduct City business in my automobile.

I understand that nothing in written materials provided to me by the City or in my communication with any City employee or official is intended to create an employment contract between the City and me. Accordingly, either the City or I may terminate my volunteer status at will at any time with or without cause or notice.              

I agree to abide by the procedures outlined in the Volunteer Handbook while volunteering at the City of Fort Collins.

 

I further acknowledge and agree that while volunteering for the City of Fort Collins I will not be an agent, servant or employee of the city, will not be entitled to any wages or compensation of any sort, and will not be covered by the city’s employee benefits, including worker’s compensation. Further, nothing in the Volunteer Handbook shall be construed to create an employment relationship of any nature whatsoever.
